Attention Incoming Freshmen:

The School Code of Illinois requires that each student entering 9th grade have a physical examination completed by a licensed physician, advanced practice nurse, or physicians’ assistant. The physical exam must be on the State of Illinois Certificate of Child Health Exam form and up-to-date immunizations compliant with the Illinois Department of Public Health. Your child will not be given a class schedule or register and begin the first day of school, without this your student will not be able to register for the upcoming school year.

Attention Incoming Seniors:

Students entering 12th grade are required to receive the Meningococcal Conjugate Vaccine (MCV4). The immunization requirement consists of two doses for the 12th grade school year, unless the first dose was administered to a child who is 16 years or older, in which case only one dose is required at the 12th grade entrance. Proof of this requirement must be turned in to the Nurse office in order for your child to receive a schedule and register for the school year.
